One $500 scholarship will be awarded to a former Bennett student who is a senior at one of the 5 Bellevue High Schools, who lives within the Bellevue School District, and whose parents are members of the PTSA.  The winner will be notified by May 23, 2004.

STUDENT PROFILE:

1.      Brief personal statement (1-2 paragraphs) that will help us to know you and how this scholarship will assist you.

2.      School Activities and Community Service.

3.      Additional information as applicable:  employment, special achievements, awards and letters, interests, hobbies, talents, skills.

4.      Essay of your choice. (A copy of a college application essay or school essay may be used.)

5.      Copy of high school transcript, including the first semester of grade 12.

6.      Plans for the future – educational or occupational goals.

7.      Schools that you have applied/been accepted to.

8.      Two letter of recommendation, one school affiliated staff member and one other of your choice.  The letters should each be in a sealed envelope and signed across the back of the envelope over the seal by the person who wrote the recommendation.

I give my permission for the contents of this application to be reviewed by the scholarship screening committee as well as the members of the Bennett PTSA Board.
